Eligible class members have several benefit options to choose from.

First, they can file a claim for a cash payment of up to $3,500 for documented losses related to fraud or identity theft resulting from the data breach.

Documentation such as receipts, emails, and phone records must be included with the claim form.

Second, class members without documented losses may file a claim for an alternative cash payment.

The amount of this payment is estimated at approximately $75, but may increase or decrease pro rata depending on the total number of claims filed.

Third, all eligible class members are entitled to one year of free credit monitoring, regardless of whether they file a claim for cash payment.

An activation code for this service should be included in the settlement notice sent.
